e-Safety Agreement

All pupils use computer facilities including Internet access as an essential part of learning, as required by the National Curriculum. Both pupils and their parents/carers are asked to sign to show that the e-Safety Rules have been understood and agreed.

Administration of Medicine

If you wish staff to administer medicine to your child during the school day, please complete this form and return it with the medication to a member of staff. A consent form should be completed each time there is a request for medication to be administered, or there are changes to medication/administration instructions.
